What does this information mean?

It's only accessible at the last stage of The Screaming Mace. Only one in the group need to be at that stage to zone the whole group in.

There is only one small chamber with 4 totems. Clicking the correct one will spawn Gorfuzod the Keeper, clicking any other one will spawn 4 harbinger of Fear. Examining Congealed Blood Sphere will highlight the proper idol.

Once the Keeper is dead, you can pick up the Screaming Mace to finish the quest.
